Alabama Medical Waste Compliance

Alabama regulates medical waste through the Alabama Department of Environmental Management (ADEM), whose Land Division runs the Medical Waste Program under ADEM Admin. Code Division 335-17, issued under the Solid Wastes and Recyclable Materials Management Act (Code of Ala. 1975, § 22-27-1 et seq.). The rules were last amended effective August 14, 2025.

Here's what the rules mean for a typical Alabama facility:

  • Register as a generator. Hospitals, clinics, physician and dental offices, labs, veterinary clinics, nursing homes, home health agencies, EMS and funeral homes are all medical waste generators. Each must register with ADEM on Form 410 within 30 days of starting operation (Rule 335-17-8-.01). There's no fee.

  • Keep a written plan. Rule 335-17-2-.01(2) requires a written medical waste plan covering the waste you generate, packaging and labeling, your transporter's name, address, phone and ADEM permit number, the treatment facilities used, pickup frequency, and staff training.

  • Accumulation limits. Once a shipping container is prepared, generators of 220 pounds or more per month may hold it for no more than 30 days. Smaller generators may hold it up to six months. Accumulation areas need biohazard signs and cleanable surfaces.

  • Packaging and labels. Outer packaging must be red or use red lettering, be labeled "Medical Waste," "Infectious" or "Biohazardous" or carry the biohazard symbol, and show your name and address and the date it was packaged. Sharps go in sealed, leakproof, rigid, puncture-resistant containers (Rule 335-17-3-.01).

  • Permitted transporters only. No one may haul untreated medical waste in Alabama without an ADEM Medical Waste Transporter Permit (Rule 335-17-5-.01), and ADEM says generators should confirm their transporter's permit is current. Permits last three years.

  • No untreated disposal. Untreated medical waste can't be disposed of in Alabama (Rule 335-17-7-.01), and it must reach treatment within 14 days of pickup.

  • Hazardous pharmaceuticals. Alabama has adopted EPA's hazardous waste pharmaceuticals rule at 335-14-7-.16 (effective April 13, 2020), including the ban on flushing them down the drain.

EnviCare holds ADEM Medical Waste Transporter Permit TRN081324-GA01 and appears on ADEM's published list of permitted medical waste transporters. We haul Alabama waste to a permitted treatment facility in Georgia, and every pickup comes with an electronic manifest that lists the package count, the treatment facility's name, address and phone number, and the shipment date.

OSHA and HIPAA Training and Plans

On top of state rules, inspectors usually ask for four documents first. EnviCare's compliance platform covers HIPAA risk analysis and policies, OSHA Bloodborne Pathogens training and an Exposure Control Plan, OSHA Hazard Communication training and a written plan, and a medical waste plan, all tracked in one dashboard.

This section is a summary, not legal advice. Confirm your facility's obligations with ADEM.

How Medical Waste Pickup Works

  1. Call or request a quote. Tell our Client Care Team what you generate, how much, and where.

  2. Get the right containers. We set you up with biohazard boxes or reusable containers, reusable or disposable sharps containers, pharmaceutical and trace-chemo containers, and shredding bins as needed.

  3. Choose your schedule. Weekly, every other week, monthly or on call, set to your volume and Alabama's accumulation limits.

  4. We pick up and document. Our drivers collect your sealed containers, and you get an electronic manifest after every pickup. High-volume sites and large cleanouts get tractor-trailer capacity.

  5. Treatment and records. Your waste goes to a permitted treatment facility in Georgia, and your manifests and certificates of destruction are available whenever you need them.

Alabama Medical Waste FAQ

Do you serve all of Alabama?

Yes. EnviCare serves Alabama statewide, from Huntsville and the Shoals through Birmingham and Montgomery to Dothan and Mobile. We also have a local CareExchange in Birmingham.

Who regulates medical waste in Alabama?

ADEM's Land Division runs the Medical Waste Program under ADEM Admin. Code Division 335-17. OSHA and the U.S. Department of Transportation also have requirements for handling and shipping medical waste.

Does Alabama require medical waste transporters to be permitted?

Yes. Anyone hauling untreated medical waste needs an ADEM Medical Waste Transporter Permit, renewed every three years. EnviCare holds permit TRN081324-GA01 and is on ADEM's published transporter list. Put that number in your written medical waste plan.

Do Alabama medical offices have to register with ADEM?

Yes. Every medical waste generator must register with ADEM on Form 410 within 30 days of starting operation. There's no fee, and each separate practice in a shared building registers on its own.

How long can we store medical waste before pickup?

If you generate 220 pounds or more per month, no more than 30 days after a shipping container is prepared. Smaller generators can hold it up to six months. We set your pickup schedule to stay inside those limits.

We have locations in Georgia or Tennessee too. Can one company handle all of them?

Yes. EnviCare serves Alabama, Georgia, Mississippi, Tennessee, North Carolina, South Carolina and Louisiana statewide, plus North Florida, so multi-state groups can use one provider.
