South Carolina Medical Waste Compliance

South Carolina calls medical waste infectious waste and regulates it under the South Carolina Infectious Waste Management Act (S.C. Code § 44-93-10 et seq.) and Regulation 61-105, last amended May 23, 2025. The program is run by the South Carolina Department of Environmental Services (SCDES), Bureau of Land and Waste Management. SCDES took over the environmental programs of the former SC DHEC on July 1, 2024.

Here's what the rules mean for a typical South Carolina facility:

  • Register as a generator. Every in-state generator must register with SCDES, each site separately, and renew every three years (R.61-105 Section F). Annual fees depend on monthly volume: none under 50 pounds, $150 for 50 to 999 pounds, and $600 for 1,000 pounds or more.

  • Small quantity generators. Facilities producing less than 50 pounds a month still register and must manage sharps, cultures, and blood and blood products under the rules (Section G).

  • Packaging and labels. Sharps go in rigid, leak-resistant, puncture-resistant containers, and inner bags must be red or orange (Section I). Each outer container needs the biohazard symbol, your SCDES generator number, and the date it went into storage or was shipped (Section J).

  • Storage limits. Generators may store infectious waste on site for no more than 14 days, or 30 days if kept at or below 42°F (Section K).

  • Manifests. Every off-site shipment must travel with a manifest on SCDES Form 2116 or another SCDES-approved form, signed by the generator, and the generator keeps a copy (Section M).

  • Registered transporters. Transporters must register with SCDES every year and put their SCDES transporter number on each manifest (Sections N and O).

  • Hazardous pharmaceuticals. South Carolina has adopted EPA's hazardous waste pharmaceuticals rule (R.61-79.266 Subpart P), including the ban on flushing them down the drain.

EnviCare is a registered South Carolina infectious waste transporter (SCDES registration IWT000008) and appears on SCDES's published list of infectious waste transporters. All South Carolina waste is treated at EnviCare's permitted treatment facility in Georgia, which South Carolina allows as an out-of-state destination facility. Every pickup comes with an SCDES-approved electronic manifest that carries all the Section M fields and generator certifications, so it meets South Carolina's manifest rule (R.61-105 Section M).

OSHA and HIPAA Training and Plans

On top of state rules, inspectors usually ask for four documents first. EnviCare's compliance platform covers HIPAA risk analysis and policies, OSHA Bloodborne Pathogens training and an Exposure Control Plan, OSHA Hazard Communication training and a written plan, and a medical waste plan, all tracked in one dashboard.

This section is a summary, not legal advice. Confirm your facility's obligations with SCDES.

How Medical Waste Pickup Works

  1. Call or request a quote. Tell our Client Care Team what you generate, how much, and where.

  2. Get the right containers. We set you up with biohazard boxes or reusable containers, reusable or disposable sharps containers, pharmaceutical and trace-chemo containers, and shredding bins as needed.

  3. Choose your schedule. Weekly, every other week, monthly or on call, set to your volume and South Carolina's storage limits.

  4. We pick up and document. Our drivers collect your sealed containers, and you get an electronic manifest after every pickup. High-volume sites and large cleanouts get tractor-trailer capacity.

  5. Treatment and records. All South Carolina waste goes to our permitted treatment facility in Georgia, and your manifests and certificates of destruction are available whenever you need them.

Does South Carolina require medical waste transporters to be registered?
